Our online questionnaire provides the highest levels of security, complying to NHS guidelines on data security and also to the American 'gold standard' for collecting Healthcare Information; the HIPAA. Any data you enter is encrypted, completely confidential and accessible only by us.
If you would prefer not to use the online service, we also have a paper version of the questionnaire. The online service is quicker to complete though and uses dynamic questioning to gain more information in the areas relevant to you. We receive your information immediately and it saves you the cost of a stamp and a trip to the post-box!
In a small number of cases it may be necessary for you to see one of our Consultants in the Anaesthetic Pre-operative Assessment Clinic. This depends on the type of surgery you are having and on your overall health. The clinic gives us the opportunity to explain the anaesthetic options in more detail to you and also to arrange any tests that may be necessary before your operation can go ahead.
If you are considering surgery but are unsure whether to proceed, you may find it helpful to see us beforehand in the Pre-Assessment clinic. We can discuss any health or medical concerns you may have and decide what the safest options are. You do not have to be booked for surgery to visit our clinic, just contact us to arrange an appointment. There is a charge for this service.
